RETEST SPECIMENS CURRENTLY AT THE LABORATORY
If you need to add test requests to a specimen that has been reported, please fax a completed Retest Request Form Form to the Redwood Toxicology Laboratory Client Services Department at (707) 577-0365.
Test requests will not be made until a signed copy is returned via fax, this form must be signed by authorized personnel. Appropriate charges will be applied to your account based on the test services requested. [Download PDF ››]
